For fluid and short term infusions up to 29 days

Contact IV team if patient from outside hospital is admitted with this line to Identify as midline.

Flush with 10ccs normal saline before and after medication doses, followed by 2ml of 10 unit/ml heparin.

Perform central line dressing change per protocol every 7 days.

Application of heat to the extremity – remember this is not the vena cava, the peripheral veins are more susceptible to venospasm

Gently traction on the catheter (without removing the dressing) - the tip of the catheter may be against the vessel wall

Movement of arm had proven effective (pronate, flexion of the elbow, 90 degree angle form body, etc.)

Gentle message near the midline catheter – under the arm, not above the expected catheter termination

PICC/Midline Extension Tubing  ( applied at home or OSH )


Remove when patients are admitted  (tubing is not stocked in medical center )

Extension may be applied by home infusion nurse prior to discharge

If patient is to be discharged with medication running (prior to home infusion nurse teaching) the bedside nurse may be asked to apply extension if left at bedside . 

Directions for application of extension tubing:

      1. Assess sealed packet for expiration date. Prime extension tubing with 3 mL 0.9% saline
      2. Change One-Link cap on PICC or midline hub per guideline
      3. Apply extension tubing directly to One-Link cap. Flush extension to assess patency and clamp Coil and tape to secure extension to skin

A decision has been made by the EP physicians to use air eliminating filters in patient's IV lines post procedure for any patient who has had a transeptal puncture from RA to LA . These IV filters would remain on until the patient was discharged. This includes the following patients;

  • Pulmonary Vein isolation PVI
  • Left atrial ablation cases

    In addition the Tandem Heart interventional patients will now have air eliminating filters placed on all lines

  • High risk cardiac cath interventional patients where Tandem Heart was used intra procedurally
  • Percutaneous Tandem Heart patients receiving continued support

    The filter to be used for air elimination is the Non DEHP Extension Set with 1.2 micron downstream filter - 2C1103. Distribution item order # J110212 and stocked on med-surg floors

Fibrin SheathFibrin Sheath

A fibrin sheath can cause infusates to track up along extra luminal sides of catheter and extravagate infusion through Insertion site.

In the event that this occurs, notify physician and plan to either replace or strip catheter of fibrin.

Fibrin Tail

If not totally dissolved, can continue to build up and eventually transform into Fibrin sheath interfering with blood draws and infusion.

Proper installation of TPA over appropriate length of time helps to avoid increase of fibrin tail
